The Questionnaire Module is an important component of all of the Medgate software suites. In the Occupational Health Software Suite, you can use this module to create and track hundreds of medical-related questionnaires. The questionnaire module also integrates with many modules in the Occupational Health Software Suite to simplify and streamline workflow.

This module will enable you to:
- Create questionnaires for any purpose; medical consent questionnaires, health history questionnaires, or exposure group specific questionnaires i.e. Bensene
- Track company-specific data that isn’t logically tracked elsewhere in the application
- Attach questionnaire responses to any other record in the application
- Gather information from across your workforce using a simple online questionnaire link
- Analyse questionnaire data using Medgate’s Business Intelligence tools
How does the Questionnaire Module Work?
The Questionnaire module offers the following features:
Creating & Using Questionnaires
- Define any type of question and related response type (Y/N, Numeric, Multiple Choice, Text, Date or Table look up) that can be added to a questionnaire
- Classify the questions into categories for easy reference
- Create a question library so that you can add the same question to many questionnaires
- Add headers/footers, company logos and default answers to a questionnaire
- Attach images to any question within a questionnaire
- Create complex formulas inside questionnaires to support/simulate various OH&S assessment tools
- Attach questionnaires to any record in the Medgate system
- Update existing questionnaires at any time with new questions or removal of questions that no longer apply
- Deliver questionnaires to employees by printing, exporting into excel and e-mailing, or sending a standalone link for online submission

Online Questionnaires/Questionnaire Inbox
- Send employees a standalone link to respond to a questionnaire and that is submitted directly to the Medgate system (they will not have access to any other record in the system)
- View all questionnaire responses for questionnaires you have access to from your questionnaire inbox
- Before posting the questionnaire, you can review and make changes as necessary

- Analyse responses by either the source questionnaire or individual question, with the option to graph the data
- Insert complex formulas into your questionnaires so that you can perform sub-section scoring
- Manipulate/graph questionnaire data using Medgate’s Business Intelligence tools
